Doug from some Van Halen museum shows it in this video briefly (video starts at the point):



Doug states it is a prototype and it is a Gibson pickup that came out of one of Ed's guitars (rewound by Seymour).

The "Seymourizer" sticker was put on pickups that Seymour rewound in the mid to late 70s. He stopped placing those before the 80s or early 80s theoretically.

Also notice Seymour's signature at top.

Could this be the SH-5 Custom prototype? Maybe.
